A handicap parking permit may be issued to you and used for up to 12 weeks after your surgery. Many local village / city halls can assist you with acquiring a handicap permit/placard. Different cities and states have different forms to complete in order to obtain a handicap parking permit. Talk to your doctor about acquiring the appropriate form to complete in order to receive a handicap parking permit.

Please be aware that misuse of the handicap permit privilege will not be tolerated by police agencies. Therefore, please refrain from using the permit when the need is no longer appropriate. Thank you for your cooperation.
